Application Materials

The entire completed packet must be submitted before formal review. The following hard copies must be mailed or faxed to the Community Partnering Program staff.

Required Application Checklist:

• Copy of completed Online Application Form.



• W-9 Federal Tax Form: please complete, print name, sign, date and return.



• IRS Non-profit Status Form: view sample.



• Liability Waiver: complete, print name, sign, date and return.



• Conflict of Interest Statement; complete, print name, sign, date and return.

If a permit is required, enclose copy of authorized document (city, county, state, federal).
